Triggers to implement Team Based Care Model

did anyone develop triggers on when to implement Team Based Care Model (ie # of pts or staff, resources available etc). As well did documentation change to support the new model?

At our institution, we decided to activate additional ICU team after 15 COVID ICU patients, each additional team will take care of approx 12-15 patients in ICU, each team will include one intensivist with 2-3 mid level support staff - APPs, fellows, residents , ?non-critical care providers if available. This is a work in progress and changing daily depends on resource and demand. We have decided intensivist will do addendum on all notes and oversee care provided for COVID ICU patients. I hope this helps.
